Care that comes to them
Dr. Potermin sees your loved one right where they live. No ambulette, no long wait in a clinic, no day lost to a stressful trip out of the building. For an older adult, avoiding that disruption is itself good medicine.
A guide for families and guardians
Choosing care for a parent or loved one in assisted living is hard, and you can't be there every moment. Our job is to be the steady medical presence in the building — so you have one less thing to worry about, and someone to call when you do.
Dr. Potermin sees your loved one right where they live. No ambulette, no long wait in a clinic, no day lost to a stressful trip out of the building. For an older adult, avoiding that disruption is itself good medicine.
If something comes up between scheduled visits, Dr. Potermin is reachable — including by telemedicine — so he and the facility's nurse can assess your loved one quickly, without waiting for the next round. Often that's the difference between a calm same-day plan and a midnight trip to the ER.
For residents with ongoing conditions, we offer Chronic Care Management and Remote Patient Monitoring. In plain terms: Dr. Potermin checks in every month — reviewing medications, watching key numbers like blood pressure or weight, and coordinating care — so small changes get noticed early, before they become emergencies. You're welcome to be part of those conversations.
You may hear from Dr. Potermin by phone each month. These services are billed to Medicare and will appear on the Explanation of Benefits. As with most Medicare services, there may be a copay or coinsurance unless it's covered by Medicaid or a secondary policy. Enrollment is entirely your choice, and you can opt out of any program at any time, for any reason.
Telemedicine services are subject to provider availability and state licensure.
Call us at (847) 354-8086 or email doctor@potermin.com. We're happy to walk you through any of this and answer questions about your loved one's care.
For familiesThese services are billed to Medicare monthly or when performed and will appear on your Explanation of Benefits (EOB). You may owe standard Medicare cost-sharing (a copay or coinsurance) unless you have Medicaid or a secondary policy that covers it. Only one practitioner can furnish and bill CCM for you in a given calendar month.
You have the right to opt out of any individual program at any time (effective at the end of that calendar month) by notifying Aspen Medical Group by phone or in writing; opting out of one program does not affect the others.
Some services (CCM, RPM, PCM) may involve standard Medicare cost-sharing (a copay or coinsurance) unless covered by Medicaid or a secondary policy.
