WHAT SETS US APART

Four things you can count on

  • Physician-led — every visit is with the doctor, not a mid-level
  • On-site — no transport, no lost day, no disruptive trip
  • Catch early — fewer changes become 911 calls
  • Continuity — the same doctor, every time, not locums

Individual results vary and depend on each resident's clinical condition; this is not a guarantee of outcomes.

For facilities

Your residents see a physician — not a rotating mid-level meeting them for the first time.

I've spent 25+ years on-site in assisted living and memory care. I see your residents myself — no mid-levels in between — and when something is off, I'm the one making the call.

We're built to catch the change before it becomes a 911 call.

On-site visits, telemedicine, and remote monitoring between visits mean a status change in the afternoon can get a clinical decision the same day — not an ER trip at midnight.

The same doctor, every time — and one who actually picks up.

Same physician, same after-hours number. We get to know your building, answer your nurses' questions about our patients, and follow up with our residents after a hospital stay.

What we bring to your building

  • On-site physician visits
  • Telemedicine between scheduled visits
  • Chronic Care Management (CCM) & Remote Patient Monitoring (RPM)
  • Behavioral health & psychiatric consultation
  • Transitional care after hospital discharge

FOR YOUR STAFF

What changes for your nurses

  • One number, the same doctor — every time
  • Faster clinical decisions, fewer disruptive transfers
  • Newer nurses can call with questions about our patients
  • Tighter documentation and medication oversight — supports your survey readiness

BEHAVIORAL HEALTH

Medicine first, psychiatry when needed

  • Agitation, depression, and dementia behaviors drive avoidable transfers
  • A physician evaluates the whole picture first — these often have a medical cause
  • A consulting psychiatrist works directly with the physician when specialized care is needed

CONTINUITY

We follow residents through transitions

  • Follow-up after hospital discharge
  • Monthly CCM check-ins by Dr. Potermin
  • Fewer loose ends on the way back

Individual results vary and depend on each resident's clinical condition; this is not a guarantee of outcomes.

GETTING STARTED

Simple, and low-risk

  • A consent packet we already have ready
  • We begin seeing residents on a regular schedule
  • You keep your existing workflows
  • Start with a few residents and grow from there

A guide for families and guardians

A doctor who comes to your loved one — and one you can reach.

Choosing care for a parent or loved one in assisted living is hard, and you can't be there every moment. Our job is to be the steady medical presence in the building — so you have one less thing to worry about, and someone to call when you do.

Care that comes to them

Dr. Potermin sees your loved one right where they live. No ambulette, no long wait in a clinic, no day lost to a stressful trip out of the building. For an older adult, avoiding that disruption is itself good medicine.

Quick access between visits

If something comes up between scheduled visits, Dr. Potermin is reachable — including by telemedicine — so he and the facility's nurse can assess your loved one quickly, without waiting for the next round. Often that's the difference between a calm same-day plan and a midnight trip to the ER.

An extra set of eyes every month

For residents with ongoing conditions, we offer Chronic Care Management and Remote Patient Monitoring. In plain terms: Dr. Potermin checks in every month — reviewing medications, watching key numbers like blood pressure or weight, and coordinating care — so small changes get noticed early, before they become emergencies. You're welcome to be part of those conversations.

What to expect

You may hear from Dr. Potermin by phone each month. These services are billed to Medicare and will appear on the Explanation of Benefits. As with most Medicare services, there may be a copay or coinsurance unless it's covered by Medicaid or a secondary policy. Enrollment is entirely your choice, and you can opt out of any program at any time, for any reason.

Telemedicine services are subject to provider availability and state licensure.
